Vecka 39-2013: Om att sluta röka

Vivi: Vi har talat om att problem kan ligga olika djupt och du gav som exempel rökning. Kan man bara träna bort rökbehovet om det inte har några djupare rötter?

Peter: Ja, om rökningen uteslutande är en dålig vana är det som regel inte svårt att träna bort den. Då leder det inte till mer än fysiska abstinensbesvär.

Vivi: Men har inte rökbehovet alltid en funktion att fylla, liksom andra symptom?

Peter: Jo. Röker vi gör vi det för att vi upplever att rökningen ger oss något. Även kroppen blir ju efterhand beroende av nikotinet. Eftersom rökningen fyller ett psykologiskt behov är det i den meningen konstruktivt att röka. Men rökningen kan även ha en funktion att fylla på ett djupare plan. Vill du sluta kan du därför behöva finna fram till det som ligger till grund för rökbehovet. Därefter gäller det att hitta något nytt och bättre sätt att fylla detta behov som inte är lika skadligt för kroppen som nikotin och som kan konkurrera ut rökningen.

Vivi: Berätta hur det kan gå till.

Peter: Det enda sättet att sluta är helt logiskt att du hittar något

• som upplevelsemässigt på något sätt ger dig en större tillfredsställelse än att röka och

• som inte kan tillfredsställas om du inte slutar röka.

Vivi: Kan det räcka med bara samtal?

Peter: Ja, ibland kan jag bara genom att samtala med min klient hjälpa henne att finna fram till nya mer positiva beroenden såsom motion, ändrade kostvanor och en ny sundare livsstil som kan konkurrera ut rökningen.

Vivi: Men om det inte räcker?

Peter: Ja räcker inte samtal hjälper jag min klient att bygga upp nya s.k. målbilder och förankra dem i hennes inre. Målbilder är föreställningar av hur du vill må och fungera här och nu och i framtiden. De tjänar som ett slags ledstjärnor som du sedan orienterar dig efter i livet. Men för att det ska räcka krävs det att rökningen bara är en vana som inte bottnar i några djupare behov.

Vivi: Och om den gör det?

Peter: Då får vi bygga upp nya jagstrukturer, t.ex. genom olika typer av självförtroendeträning, som jag sedan kan ”hänga upp” eller integrera de nya målbilderna i.

Vivi: Vad menar du med ”hänga upp”?

Peter: Föreställ dig att du brutit en tand ända nere vid roten. Då finns det inte något att fästa en fyllning eller krona i. Därför behövs det en brygga att fästa den nya tanden i. På motsvarande sätt kan man behöva bygga upp jaget för att ha något att fästa målbilderna i.

Vivi: Jag förstår. Och om det inte räcker?

Peter: Då kan vi sluta oss till att även bearbetning behövs för att målbildsträningen ska fungera. Då glider träningen över i terapi.

Vivi: Kan du sammanfatta förutsättningarna för att det ska räcka med självhjälpsträning.

Peter: Ja självhjälpsträning räcker då

• rökningen bara är en vana eller egentligen ovana och inte har några djupare rötter i en persons behovshierarki, d.v.s. inte fyller basala behov t.ex. av att känna sig älskad och bekräftad och att känna en grundläggande inre trygghet


• personen ifråga dessutom lärt sig att komma in i ett förhållandevis djupt hypnotiskt tillstånd, och

• har jagstrukturer som kan bära upp de nya behov som bl.a. genom målbildsträning utvecklas och som ersätter och samtidigt utesluter rökningen.
